Upon completion, participants will be able to:
? Differentiate between a quality documentation entry versus one that is considered to be a poor entry.
? State at least three ?do?s? and three ?don?ts? that should or should not occur in documentation entries.
? Explain how defensive charting advocates for an individual in legal situations.
? Describe how documentation is related to charting the progress of the patient?s course of treatment in the health care facility.
? List at least three documentation guidelines that should always be employed when documenting in the patient record.
? Identify at least two types of events that should be documented in the patient record.
? Describe the role of patient/family/significant other comments used as direct quotes in the patient record
