In recognition of National Healthcare Decisions Day on April 16th, Hospice of the Upstate will be conducting a class in the two statutory forms provided by the State of S.C. for providing written instructions on life-sustaining medical procedures;

1. The Declaration of a Desire for a Natural Death
(the so called ?living will?)
2. S.C. Healthcare Power of Attorney.

A free copy of the forms will be given to each participant.
Classes are free and open to the public.
