April 16th is National Decision Day!

Please join Elite Day Center and Wren Hospice and make your healthcare decision known with the Five Wishes program.

The program Five Wishes meets the legal requirements for an advance directive in ​​South Carolina. You can use Five Wishes in ​South Carolina to express how you want to be treated if you are seriously ill and unable to speak for yourself and we will be there to help with completing the Five Wishes packet, along with a notary.
