Join us Wednesday September 27th at noon for Spartanburg Upstate Senior Network Meeting.
This month Alana Lanford, CMR Coordinator for SRHS will be speaking on the Community Medication Reconciliation Program. This program was designed to address the National Patient Safety Initiative to reduce medication errors among hospital inpatients and outpatients. The overall goal of the Community Medication Reconciliation Program is to reduce medication errors by making the Universal Medication Form available from the patient's multiple points of medical access. The program hopes to accomplish this goal through the combined efforts of the CMR Coordinator, their hospital partners and the community they serve.
