As an Admissions Coordinator (i.e., Social Service Coordinator) at Lincolnton Rehabilitation Center, you will support the center’s efforts to build census, as well as coordinate pre-admissions, admissions, and the discharge process, for residents and patients in a long-term care and rehabilitation setting. You will support initiatives to develop census, track activities from referral sources, conduct building tours for potential clients and their families, prepare new patient admissions, obtain medical records and pre-authorizations, welcome new residents, complete their admission, and coordinate with social services to ensure their psychosocial needs are met. You will support the patient’s needs throughout their stay, ensuring their treatment goals are achieved and discharge process successfully completes. Required Experience
High School Diploma or equivalent required.Post-secondary education OR experience in a related field a plus (i.e., Medical Admissions, Health Insurance, Healthcare Services, Business, Marketing, Social Services, Discharge Planning, and/or Patient Education)
Must be familiar with Medical Terminology
Must be proficient in the use of personal computer (Windows, Internet, and Applications/Programs navigation)
