Position Description

Qualifications for case managers to include: (1) individuals with a
bachelor’s or master’s degree in a health or human services field (2)
Certified Case Managers. All individuals will be required to have two (2) or more years of experience in assessment and care planning with clients. Case assignments for newly hired case managers: First 30 days cannot service more than 35 cases of the initial successful training. After the 31st through 60 days, cannot service more than 55 cases. After 60 days of training, caseloads may be increased to maximum 85 cases.

Required Experience

Assessment and care planning: 2 years experience.

Able to demonstrate excellent oral and written communication skills. Must be able to assess client’s need and implement care plans. Must be able to document client’s needs and work with them in a positive way to help them to implement their service plan. Must be able to conduct home visits with diverse cultures and speak with others on
your client’s behalf, and act as an advocate for them. Must be a good
listener and able to speak in an understandable empathic way to gain and maintain rapport with clients, family and staff members.

Strong ethical principles of honesty and integrity; with the ability
to work with diverse populations within the community. Must be able to
accept directions and able to implement policy changes. Must be able to work well with others and be a part of a team member, and is also able to work without supervision with the role and scope of service.

Must have an excellent organizational and time management skills with the ability to keep appointments and manage appointment calendars, and the ability to meet multiple deadlines.

Keyboard and web navigation skills. Must have intermediate to advanced computer skills and experience with Microsoft Word, Outlook, and Excel. Must have a valid SC driver’s license and have access to a reliable transportation.
