The community care manager provides comprehensive assessments, designs individualized care plans, coordinates resources and services, monitors and evaluates the care plan for outcomes which address the health, functional, psychosocial, legal/financial and environmental needs of the client. Candidate will collect and report data and evaluate outcomes. Candidate should be highly organized and independent with excellent analytical and problem solving skills. The community care manager will maintain excellent working relationships with providers, act as a liaison and client advocate and educate clients, family members and the community. Position covers Spartanburg, Cherokee and Union County. Travel required.

Registered Nurse, BSN, MSW, or LMSW or other human service related masters degree with a minimum of 3 years experience in healthcare, community-based healthcare and/or long-term care (e.g. home health, skilled nursing home care, hospital discharge planning). Candidate should be knowledgeable in geriatric care/issues. Prefer advanced certification such as Certified Care Manager, Geriatric Care Manager, Certification in Gerontology, etc. Must demonstrate clinical competency with evidence of ongoing professional educational training. Candidate should demonstrate excellent verbal and written communication skills. Ability to present one self professionally in the community.
