CUSTOMER SERVICE NURSING / INTAKE COORDINATOR / LPN

The customer service representative is required to collaborate with patients, families, staff members, physicians and vendors in a professional and confidential manner in regards to customer service.
The LPN follows the establish plan of care that the Nurse
Case Manager and the IDT members approve. He/She assesses the patient for continuing appropriateness for hospice care, and changes in the plan of care, following accepted standards and nursing practices.

1. Coordinates assessment and admission of all potential patients.

2. Takes all initial phone inquiries regarding patient admission.

3. Arranges for assessment of potential patients by a CHC representative,

4. Negotiates financial arrangements with insurance providers and documents terms of agreement.

5. Communicates all billing information to Accounting Department.

6. Communicates insurance information and need for update to CHC billing Department.

7. Processes Medicaid/Medicare admissions.

8. Provides clinical coordinator with information and dates for recertification.

9. Arranges for the completion of all paperwork required for admission (Orders, certificate of terminal illness, legal documents, Medicaid forms, etc.).

10. Notifies referral source of admission and or non-admission

1. A current south carolina LPN license, with at least 1 years of experience in the field.

2. Familiarity with a variety of the clinical concepts, practices, and procedures.

3. Ability to exchange routine information in an appropriate manner.

4. Good oral and written communication skills
