Job Responsibilities:
• Direct and review applicable written policies and procedures that support the specialized memory care program’s mission statement and goals.
• Provide and/or oversee initial and ongoing training of all memory care program staff including (a) Dementia Capable Care training and (b) Warchol Best Abilities Care Model℠ training.
• Facilitate and assure memory care staff competencies in delivering Dementia Capable Care and operating the Warchol Best Abilities Care Model℠.
• Coordinate and align care programs (ADLs, activity, therapy, dining) and required documentation and staffing to support the specialized memory care program’s mission statement and goals.
• Assure staff scheduling reflects life story preferences of the residents.
• Assure activity and daily care delivery reflects life story preferences and resident cognitive level.
• Assure personnel, residents, families and visitors and attending physicians follow established policies and procedures.
• Meet or talk with families/prospects during inquiry about the specialized dementia program. Provide information about the care model, engage in discovery about the prospect, and provide resource information as appropriate.
• Perform pre-admission assessment(s), in accordance with the Warchol Best Abilities Care Model℠ process, for persons requesting admission to one of the specialized memory care households. Coordinate and review the pre-admission information and assessments with appropriate team members prior to admission and determine appropriate level of care/household.
• Responsible for direct supervision of Activity Manager and Care Partners.
• Recruit, screen and train memory care program leadership and other memory care personnel in cooperation with appropriate department heads and in accordance with facility and program policies and procedures.
• Facilitate and/or oversee the competency development of memory care staff to the required level of competence and confidence, as related to specific job descriptions.
• Regularly mentor staff and model delivery of dementia capable care and Warchol Best Abilities Care Model℠ in accordance with all policies and procedures and assist staff with problem resolution.
• Facilitate and lead or co-lead Dementia Capable Care Committee/QI meetings, family educational forums/support groups and resident/family council meetings
• Follow up on all memory care facility residents hospitalized for more than 24 hours by visiting the hospital or supplying dementia specific caregiving information to the hospital staff. Follow up with family and provide progress reports to the program staff.
• Be available and support resident families and significant others as partners in care throughout the entire care process and journey.
• Be available to families and staff when a resident crisis or death has occurred. Offer support to participant’s families, when appropriate, by attending services for the deceased.
• Work closely with all facility managers to promote an exceptional interdisciplinary specialized dementia program as evidenced by the development, implementation and monitoring of quality improvement/assurance measures to ensure program accountability.
• Maintain awareness of best practices and research advancements and integrate as appropriate in specialized dementia program and teachings
• Present at least semi-annually, publish, and/or provide educational programs for community organizations, professional organizations and others, therefore establishing a positive, leadership reputation in the local community.
• Participate in marketing/outreach activities representing the memory care program including networking with other health care professionals and influencers, and working closely with the Alzheimer’s Association, both nationally and locally, as a liaison between the facility and the Association.

Base salary range 40K-45K DOE

Education: Degree, preferably in Occupational Therapy, Social Services, Gerontology, Nursing, or Recreational Therapy

Experience: At least 2-3 years of leadership and caregiving in dementia care, managerial/supervisory experience, and presentation skills and experience.
