INTRO: What do we tell clients an Elder Care Coordinator does?
A holistic life care plan starts with a legal and financial foundation that will provide for peace of mind as you begin the Life Care Planning (LCP) journey with your loved one. The path is illuminated by expertise and support of an Elder Care Coordinator (ECC). This advocate will work closely with you as part of the LCP team. Their experience and expertise in home care, social work or geriatric care management will help them guide you with a comprehensive approach to developing a care plan for your loved one through assessment, education, coordination and advocacy. As a supportive and knowledgeable professional your ECC will coordinate your care by referring you to qualified local care providers, facilities, counselors and support groups. The ECC will advocate for your care needs so you can remain in your home as long as possible. By personally visiting the home, they are able to assess safety issues and recommend solutions like transportation programs and stay up to date on technology to improve quality of life. They identify quality and cost of resources in the community allowing you to make better decisions. When remaining in your home is no longer an option, your ECC will attend care plan meetings and be a zealous advocate for your loved one to ensure the best possible care in the most appropriate environment. Your ECC keeps you informed on patient’s rights, responsibilities of care needs, and what to expect from care providers or when a new level of care is needed, like end of life care. We know that changes occur and when they do your ECC will collaborate with the LCP team to revise the LCP and to assure your loved one receives the quality of care they deserve through all stages of aging. REQUIREMENTS:
- Travel to visit clients, facilities, networking events, workshops, LL offices across the state, etc.
- A solid knowledge of the senior living industry in local and surrounding areas. The candidate will understand the challenges seniors face, have connections with local resources, and have a passion for serving seniors and helping them plan for their future.
JOB DUTIES:
- Participate in client consultations.
- Visits and assessments with clients- capacity, ADLs, and safety.
- Assist with placement options for all levels of care, to also include in-home care.
- Refer clients to local resources related to making progress on their LCP.
- Participate in postmortem consultations to determine any next steps or closing of the file.
- Participate in community networking, relationship building, business development.
- Public speaking at workshops, senior classes, networking groups, etc.
- Accurate and timely tracking of client activity in shared database.
- Follow up with attorneys on their action steps as part of the LCP.
- Making sure clients are getting their annual Medicare review done.
Required Experience
Knowledge of Topics That Are Regularly Addressed in Life Care Planning. Training will be provided, however, an awareness of these topics is helpful.- Estate planning
