Elderly Disabled Service Coordinator

The SPARTANBURG HOUSING AUTHORITY is seeing qualified candidates for this full time position to provide general case management and service coordination to low income elderly and disabled residents. The person in this position would establish linkages with agencies and service providers in the community, and will refer residents to these resources. Examples of services provided are: individual case management, referrals for home health, transportation, counseling, medical services and wellness programs. Will also coordinate and schedule monthly activities and programs. Needs to be able to establish trustworthy relationships. Good communication and Microsoft Office skills, as well as a positive attitude. Must be willing and able to drive a passenger bus, transport residents and lift up to 50 pounds.

The ideal candidate will have two years’ experience in social service delivery for senior citizens and disabled adults and / or low-income individuals. Bachelor’s degree in related field. Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities will be considered. Must have valid and clean driving record and background check.
