Position Description

Will provide case management to elderly / disabled individuals. Will be responsible for making referrals to the appropriate health care provider(s), will maintain client files, will provide educational services to residents.

Required Experience

MUST BE A REGISTERED R.N. No exceptions. Start date can be ASAP if you choose, application deadline is open until position is filled.
