Position Description

The Social Worker is an integral part of the Interdisciplinary Team and is responsible for assessing the psychosocial needs of the hospice patient and family. Understanding that psychosocial services must be delivered according to accepted standards of practice, the Social Worker, guided by the patient/family's needs and wants, coordinates the resources of the family, the Hospice and the community into an integrated program of support.

Required Experience

Qualified applicants will possess a Master's degree of Social Work from an accredited school of social work and a current SC Social Work License. One year of social work experience in a health care setting is required; experience in a Geriatric setting is preferred. Applicants should also possess a current driver's license, auto insurance and a clear driving record.
