Position Description

Experienced Social Workers needed to provide specific expertise in interdisciplinary end-of-life care to patients and families.

The position is for serving home care patients in Polk County, NC and in the northern communities of upper Spartanburg and Greenville counties.

Required Experience

SC Social Work license required. Experience in geriatrics or end-of-life care preferred.

Start dates are negotiable.
