Position Description

Provide a variety of community health programs for residents of senior living facilities within the Spartanburg Housing Authority.

The program is Project House Calls and primary responsibilities will include conducting and coordinating group and individual health education and wellness activities for a diverse seniors population. Other duties will involve acquiring a knowledge of the broader health provider community such as the Department of Health and Environmental Control, area hospitals, Spartanburg Area Mental Health Center as well as RCHC in order to manage the health and wellness of these residents across the health continuum.

The selected candidate will maintain a repository of health information for those residents who participate. Monthly oversight meetings of SHA and RCHC will be held for planning purposes, and to continually monitor the program?s effectiveness.

Required Experience

A Registered Nurse is required for this position.
