Position Description

Provide Clinical support and back up to Health and Wellness Director in Assisted Living and Memory Care Units. Rounding with facility providers and documenting treatments and follow up. Supervisory role over Memory Care Unit associates. Assist with resident assessments.

Required Experience

LPN required; Assisted Living and/or Alzheimer's & Dementia Care experience preferred; supervisory experience requested.
