Position Description

Assesses patients and families, develops care plans and provides individual and family counseling for hospice patients in the home setting.

Required Experience

Requirements include a Masters of Social Work from an accredited School of Social Work, must be licensed by South Carolina Board of Social Work Examiners; active and in good standing and eligible for Georgia Social Workers license. Two years’ experience in health care setting preferred.
