Position Summary: We are seeking a dedicated and compassionate Licensed Master Social Worker (LMSW) to join our hospice team. The Hospice LMSW Social Worker will provide emotional support, counseling, and resource coordination to patients and their families, ensuring their psychosocial needs are met during this critical time. This role requires a deep understanding of end-of-life issues and the ability to work effectively within an interdisciplinary team.
Key Responsibilities:
Conduct comprehensive psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and needs.
Develop and implement individualized care plans in collaboration with the hospice team, addressing the unique needs of each patient and family.
Provide supportive counseling and crisis intervention to patients and families, helping them cope with the emotional aspects of terminal illness and bereavement.
Educate patients and families about the hospice philosophy, services available, and the dying process.
Facilitate access to community resources, including financial assistance, support groups, and other services that can aid in improving quality of life.
Serve as an advocate for patients and families, ensuring their preferences and rights are respected.
Participate in interdisciplinary team meetings, providing insights and recommendations for patient care.
Document all patient interactions and interventions in accordance with regulatory requirements and vCare Hospice policies.
Assist with advance care planning and the completion of advance directives.
Provide bereavement support to families following the death of a loved one, including follow-up calls and referrals to grief counseling services.
Stay current with best practices and developments in hospice and palliative care through continuing education and professional development.
Qualifications:
Master's degree in Social Work (MSW) from an accredited institution.
Licensed Master Social Worker (LMSW) in the state of South Carolina.
Minimum of two years of experience in a healthcare or hospice setting preferred.
Strong understanding of end-of-life issues, grief, and loss.
Excellent communication and interpersonal skills, with the ability to build rapport with patients, families, and team members.
Ability to work independently and as part of a multidisciplinary team.
Proficiency in electronic medical records (EMR) and documentation.
Valid driver’s license and reliable transportation for travel within the service area.
Work Environment: The Hospice LMSW Social Worker will primarily work in patients' homes, hospice facilities, and occasionally in hospitals or nursing homes. This position requires travel within the Upstate region of South Carolina.
