Position Description

Hospice of the Carolina Foothills is seeking a Full Time Medical Social Worker for our SC Division. The Social Worker is an integral part of the Interdisciplinary Team and is responsible for assessing the psychosocial needs of the hospice patient and family. Understanding that psychosocial services must be delivered according to accepted standards of practice, the Social Worker, guided by the patient/family's needs and wants, coordinates the resources of the family, the Hospice and the community into an integrated program of support.

Required Experience

A South Carolina Social Work License, Master's degree of Social Work from an accredited school, and at least one year of social work experience in a health care setting are required. Geriatric experience is preferred.
