The Social Worker is an integral part of the Interdisciplinary Team and is responsible for assessing the psychosocial needs of the hospice patient and family. Understanding that psychosocial services must be delivered according to accepted standards of practice, the Social Worker, guided by the patient/family's needs and wants, coordinates the resources of the family, the Hospice and the community into an integrated program of support. This position will provide services to homecare and hospice house patients.
Required Experience
*Master's degree of Social Work from an accredited school of social work required.*Licensed Social Worker in SC required, preferred in NC.
*At least one year experience in social work, preferably in the healthcare field.
*Knowledge of and ability to apply psychosocial skills and counseling techniques in a wide variety of hospice situations.
*Knowledge of resource and support agencies and the methods of accessing services.
*Demonstrates good communication and problem solving skills
*Ability to document in a professional manner.
*Ability to conduct educational seminars and training.
*Ability to maintain confidentiality and professional conduct at all times.
*Demonstrates a commitment to the vision and values of Hospice of the Carolina Foothills
*Understands and is committed to the hospice philosophy and its concept of care.
*Driving or other mode of transporation required.
*Must maintain a current driver's license, auto insurance and a clear driving record.
*Some work outside of normal business hours may be required.
*Contact with patients/families with some risk of exposure to communicable disease.
*Tuberculin skin test required before rendering services to patient/family and annually thereafter.
