Position Description

We are seeking a Master?s level social worker with hospice experience. Responsibilities include providing supportive and medical social services to terminally ill patients and their families in accordance with an appropriate plan of care. Identifying and utilizing community resources for the benefit of the patient and family.

Required Experience

Previous hospice, home health or long term care experience preferred.
