Position Description

Requirements include a Masters of Social Work from an accredited School of Social Work, must be licensed by South Carolina Board of Social Work Examiners; active and in good standing and eligible for Georgia Social Workers license. Must have a flexible schedule. Assesses patients and families, develops care plans, provides counseling to patients and families and develops discharge plan for patients as needed.

Required Experience

Two years' experience in health care setting including discharge planning preferred.
