Patient Care Coordinator

Home Instead Senior Care is a non-medical home care company that provides companionship, home helper, personal care and Alzheimer's care to senior citizens in our community. Our goal at Home Instead Senior Care is to assist seniors to live safely in their homes.

Home Instead Senior Care is looking for a Client Care Coordinator who is an organized individual to join us in our mission to enhance the lives of aging adults and their families. The Client Care Coordinator helps to establish new clients, develop a plan of care and manage quality assurance with one-on-one visits with our clients.
The Client Care Coordinator will be expected to determine each individual client's needs to provide solutions and create a tailored service plan for the client. The Client Care Coordinator will continuously evaluate each client’s care plan through a series of ongoing communications and visits to ensure the highest quality care, client satisfaction and retention for each of our clients.

Responsibilities

Quality assurance visits with our clients
Consultations with potential clients and family members
Creating all client care plans and assessments
Managing changing client needs
Managing caregiver performance and care for our client
Client/Caregiver introductions
Qualifications

College, Nursing or Social Work Degree preferred
At least one year experience in home care, health care or senior-related industry required
Must demonstrate excellent oral and written communication skills and the ability to listen effectively
Must have the ability to work independently and meet deadlines
Must demonstrate knowledge of the senior care industry
Must have the ability to organize and prioritize daily, monthly, quarterly and yearly work
Must have computer skill and be able to use Microsoft Office products
Must have the ability to present a professional appearance and demeanor
