Position Description

The PRN Geriatric Care Manager (GCM) is available for after hours on-call rotation Monday-Thursday (5p-9a), Friday (2p-9a) and/or Saturday-Sunday 9a-9a. The GCM must be available to take phone calls during the designated times and/or travel to assist with client needs. The PRN GCM must problem solve, coordinate and efficiently execute directives for care with the on-call team. The on-call team includes the GCM, an RN and a caregiver supervisor.

Required Experience

Bachelors degree in psychology, social work, or human services field desired and at least 1 year professional experience with geriatrics
