Position Description

We are seeking an RN Case Manager with hospice experience. Minimum 1 year acute care experience required. RN Case Manager provides skilled nursing care to hospice patients as well as oversight of delivery of hospice nurse aide services. Populations include patients in skilled nursing facilities, assisted living facilities, and in home settings.

Required Experience

Minimum of 1 year acute care experience required. Previous hospice experience preferred.
