Position Description

RN Case Manager oversees, plans, directs hospice care for clients in the upstate of South Carolina. Initiates plan of care for clients, manages care provided by hospice team, participates in on-call rotation, attends interdisciplinary meeting and works with family to meet the emotional/health/spiritual needs of client.

Required Experience

Valid and current RN License in South Carolina. Hospice experience preferred but not required.
