Position Description

RN needed to do home visits with resources coordination in the Upstate, SC area.

Responsible for surveying the patient and help coordinate resources available.

Part-time position, 20-25 hours per week.

Home Visit and Care Management experience preferred.

Case load builds gradually until maximum of 10-15 patient caseload is met. Training consists of our orientation; Senior Bridge Self Study Modules and webinars and a ride along with one of our nurses.

Required Experience

Must have a current RN license in SC.

Computer skills required and home care experience a plus.
