Position Description

Coordinate care plans for skilled nursing residents including communication with families.
MDS Assessments and appropriate documentation
Discharge Planning
Coordination of durable medical equipment and services needed for patient post discharge
Must have strong written and verbal communication skills

Required Experience

Bachelor's Degree in Social Work, Psychology, Geriatrics or related field
Experience working with elders preferred
