Responsibilities:
As the Social Services Director, you'll plan, develop, direct and evaluate the operation of our facility’s social service programs in accordance with resident needs, government regulations and our internal policies and procedures. Tasks in this role include participating in discharge planning, development and implementation of social care plans and resident assessments; and providing consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of residents through the development of social service programs. Assisted Living or Skilled Nursing Facility experience preferred.
Required Experience
Position requirements include:•Licensed to practice social work in the state where the facility is located, or if licensure is not required, person with Bachelor’s degree in social work, human services, sociology, special education, psychology.
•1 year of supervised social work experience in a health care setting working directly with elderly individuals.
•Must be a member in good standing in the National Association of Social Workers and Academy of Certified Social Workers, Inc.
•Must meet all health requirements, including TB, and pass background checks.
