Position Description

This is a FT position within our Skilled nursing. Candiates must have LTC experience and a degree in Social Work or other Human Related Field such as Sociology, Special Education, Rehabiliation Counseling or Psychology. Responsibilities include Social services duties for approx. 60 beds, resident and family support, MDS/RAPS, Care plans, Advanced Directives and monitoring and facilitating resident psychosocial wellbeing, and discharge support.

Required Experience

Must have Long Term care experience
