Position Description

The Social Worker is an integral part of the Hospice interdisciplinary care team and as
such is responsible for assessing the psychosocial needs of the hospice patient and family.
Understanding that psychosocial services must be delivered according to accepted
standards of practice, the social worker, guided by the patient/family’s needs and desires,
coordinates the resources of the family, the Interdisciplinary Team of Journey Healthcare,
and the community into an integrated program of support.
Duties and Responsibilities
• Provides social work services based on patients psychosocial assessment and the
patients and family’s needs and acceptance of these services
• Implements hospice information visits as directed
• Assesses patient and family for psychosocial needs and concerns within five (5) days
after the patient’s election of the hospice benefit
• Participates with the Interdisciplinary Team in developing the team plan of care within
48 hours of admission
• Coordinates psychosocial care to meet the needs and concerns of the patient and
family in a manner that is meaningful to them
• Visits hospice patients/families regularly as defined by individual plans of care
• Submits all documentation within 24 hours or next working day
• Acts as a liaison where needed between patient/family, hospice team, and outside
resource agencies
• Serves as a member of the interdisciplinary team and attends team meetings
• Works with team members in a collaborative effort and communicates with openness
and professionalism
• Serves as counselor resource to staff and volunteers
• Responds to patient/family needs after hours when requested
• Participates in continuing education for hospice staff
• Coordinates or participates in presenting hospice education for staff, volunteers,
resource agencies, and the community at large
• Establishes working relationships with social workers, and caseworkers in outside
agencies
• Assists patients/families to prepare financial assistance applications forms
• Monitors patients/family’s financial resource availability
• Follows all appropriate policies and procedures of Journey Healthcare
• Carries out all other duties as assigned

Required Experience

Qualifications
• Must be licensed as a Social Worker by the State Board of Social Worker examiners.
(Applicable to Georgia, South Carolina & Texas only). In lieu of licensure
requirement, Louisiana Hospice Social Worker must have a master degree from a
school of social work accredited by the Council on Social Work Education.
Documented clinical experience appropriate to the counseling and casework needs of
the terminally ill. Must be an employee of the hospice
• Must have current CPR certification.
• Must possess a valid state driver’s license and current auto liability insurance.
• Must have a clear state specific motor vehicle check as applicable
• Knowledge of hospice philosophy, death and dying processes, grief, loss, and
bereavement skills, and basic counseling skills
• Possesses a reasonable degree of personal comfort with death and dying process
• Must be willing to work independently, be flexible and reliable
Knowledge and Abilities
• Knowledge and sound understanding of hospice philosophy and its concepts of care
• Knowledge of and ability to apply psychosocial skills and counseling techniques in a
wide variety of hospice situations
• Ability to respond and function appropriately in crisis situations
• Extensive knowledge of resource or support agencies and the methods of accessing
services
• Ability to conduct educational seminars and training for civic, church, volunteer and
professional groups
• Ability to conduct basic grief/bereavement counseling in individual, group, and/or
family setting
• Knowledge and understanding of need to make referral to outside professional for
issues/complications beyond the scope of hospice care
