Position Description

Skilled Nursing Facility Social Worker, In-Person, Full-Time

Required Experience

Must have a Bachelors degree in social work, or a human services field including, sociology, gerontology, special education, rehabilitation counseling and psychology. Experience in a skilled nursing facility with discharge planning a must.
