Position Description

Ark Hospice has an immediate opening for a compassionate and highly-qualified SOCIAL WORKER to join our growing team of caregivers.

The primary purpose of the Hospice Social Worker's job is to manage the social service needs of hospice patients, their families and caregivers. This includes all social service interventions and referrals, including needs assessment, supportive counseling, networking, resource procurement, crisis intervention and discharge planning. The Hospice Social Worker will serve as a part of the interdisciplinary group, attending team conferences and coordinating care with the primary hospice nurse, family and client services staff and volunteers.

Ark Hospice currently serves the Upstate of South Carolina and is expanding into other counties. Each member of its team is critical to the mission of compassionate living for every patient.

Required Experience

Requirements: Graduate of a Master?s or Bachelor?s program in social work accredited by the Council on Social Work Education; and currently licensed as a LBSW or LMSW by the State of South Carolina. Must have a minimum of one year social service experience in a health care setting. Hospice experience preferred. Computer skills required.
