Position Description

The Social Worker is an integral part of the Interdisciplinary Team and is responsible for assessing the psychosocial needs of the hospice patient and family. Understanding that psychosocial services must be delivered according to accepted standards of practice, the Social Worker, guided by the patient/family's needs and wants, coordinates the resources of the family, the Palliative Care Support Team and the community into an integrated program of support.

Required Experience

Qualifications: Bachelor's or Master's degree of Social Work from an accredited school of social work, and social work license in SC required. A minimum of one year experience in medical social work, geriatrics experience preferred.
