Position Description

Faith based retirement community is seeking a Transition Coordinator to handle Marketing, Admissions, and Discharges for a New Medicare A Short Term Rehab Unit. Person will be expected to build relationshiops with Hospital Case Managers, Discharge Planners, physicians, and community members.

Required Experience

Bachelors degree and at least one year of skilled nursing experience preferred.
