Position Description

Coordinates admissions of Medicare part A rehab patients from area hospitals and with their discharge to appropriate placement after therapy completion. This person is also responsible for social services assessment notes.

Required Experience

Medicare admission experience required
Associates degree required, Bachelors Degree preferred
