Position Description

Faith based Retirement Community is seeking a Transition Coordinator to handle Marketing, Admissions, & Discharges for New Medicare A Short Term Rehab Unit. Person will be expected to build relationships with the community as well as with Hospital Case Managers and discharge planners. Person will facilitate the Admissions process into the Med A Unit.

Required Experience

Bachelors Degree preferred
Healthcare experience is needed.
