The Hospice Aide is an integral part of the Hospice Interdisciplinary Team and as such is
responsible for providing, under the supervision of a Registered Nurse, personal care services to
patients and/or homemaker services in support of the caregivers. Understanding that personal and
homemaker services must be provided in a manner that is meaningful to the patient and family, the
Hospice Aide must reflect the hospice philosophy and concepts of care at all times.
• Reports to the RN Case Manager assigned by the Director of Nurses/Supervising Nurse
and/or Patient Care Coordinator
• Works with the entire Interdisciplinary Team
• Works and builds relationships with patient care facility staff and reflects hospice
concepts of care
Position Conditions
• High level of contact with patient/family with risk of exposure to communicable disease
• Driving or other mode of transportation required with current State driver’s license and auto
liability insurance mandatory
• Work is mostly in patients homes, which are not usually wheelchair accessible
• Reporting system required is written and/or verbal
Duties and Responsibilities
• Provides personal care and/or homemaker services to the patient as ordered by the
Interdisciplinary group and follows the plan of care as assigned by the RN on the patient
specific aide assignment sheet
• Informs registered nurse of any unexpected changes in the patient’s condition
• Reports, in writing, assignments carried out and submits documentation to be filed in the
Hospice Clinical Record
• Attends team meetings as requested
• Is responsible for monitoring the continuing education requirement (according to the state of
employment) and suggesting to supervisor programs of interest
• Is responsible for following agreed upon schedule and keeping supervisor informed of any
changes in scheduling
• Follows all appropriate policies and procedures of Journey Healthcare
• Carries out all other duties as assigned

• Must fulfill one of the following:
-A minimum of 1 year full time experience in direct patient care in a hospital or nursing
-1 year full time experience within the last 5 years in direct patient care with a hospice or
home health agency
• Completion of a state approved Hospice aide training course with current Nurse Aide
Certification in state providing patient care services
• High school diploma or GED
• Listed on the registry of aides certified by the State Department of Health with no findings
against the aide relating to client abuse or misappropriation of client property
• Successfully pass the written test provided by Journey Hospice at the start of employment and
• Pass the competency evaluation of skills performed under the supervision of a Registered Nurse
• Must have current CPR certification
• Must have a dependable mode of transportation
• Must possess a valid State driver’s license and current auto liability insurance
• Must have a clear state specific motor vehicle check as applicable
• Must be able to work independently, be flexible and reliable
Knowledge and Abilities
• Must be able to read and write English and carry out directions
• Personal care knowledge such as addressed in the written test and is necessary for quality
patient care
• Ability to get along with people to build positive relationships with patient families and facility
• Personal care techniques, which contribute to quality patient care and are implemented in a
gentle, safe manner
• Infection control techniques
• Knowledge of hospice philosophy and concepts of care
• Knowledge of the grief process
