Alzheimer’s Association Respite Application Now Online

April 1, 2008

The South Carolina Chapter provides short-term respite--time away from caregiving responsibility--for full-time family caregivers. A voucher for up to $500 worth of care is available once a year.

With your respite voucher, family members are able to arrange care through approved home care agencies, adult day care, or temporary placement in a long term care facility. The approved agency or facility will bill the association for up to $500 worth of services. The respite program is not designed to pay for services on a long term basis.

Application materials in PDF form are available to download and print. Please make sure that you print all 4 documents (6 pages total).

The four documents include:
Explanation of Application Process
Guidelines of Respite Program
Respite Application
Doctor's Statement Form

You can access the forms by going to our homepage (www.alz.org/sc) and clicking on Respite Services. You can also go directly to the site by following this link: http://alz.org/sc/in_my_community_10485.asp.

We hope this will help improve accessibility both for families and for our professional contacts who pass this information on. These forms are recently updated, so please make sure that your copy is up-to-date.

Thank you for help in spreading the word about this program!
