How do I know if the community I’m interested in is a Life Plan Community?
A Life Plan Community is a replacement name for the category currently called Continuing Care Retirement Community.
At a minimum, a Life Plan Community should:
• Offer more than one level of care on a single campus
• Have a focus on an active lifestyle
• Be integrated into the community with an emphasis on giving back and being socially responsible
A Life Plan Community follows the definition of a CCRC set by NIC as:
Age-restricted properties that include a combination of independent living, assisted living and skilled nursing services (or independent living and skilled nursing) available to residents all on one campus. Resident payment plans vary and include entrance fee, condo/coop and rental programs.
Life Plan Communities offer a wide range of housing choices and services for a life filled with possibilities while still offering a continuum of supportive and advanced health care services should they be needed. As a result, a Life Plan Community allows planning and living to merge. Having a plan in place—the security and safety net provided by the availability of health care, coupled with the freedom from not having to manage all the day-to-day tasks that come with homeownership—allows residents to live life to the fullest.
Among the many living options for older adults, a Life Plan Community is the retirement lifestyle that offers the highest level of expectations; meeting you where you are along your journey and offering support as you need it; all on the same campus. If the future holds an unforeseen decline in health, a Life Plan Community offers a strategy to address those situations while you continue to live among your friends and support group.

The Village at Summerville is the first of six Life Plan Communities of the Presbyterian Communities of South Carolina (PCSC). While serving seniors for 60 years, the community continues to listen to the wants and needs of seniors. In 2009, it became apparent that a new level of care was necessary in addition to the Independent Living, Assisted Living and Long Term Care Skilled Nursing already provided on the campus.

RENEW is PCSC’s innovative Short-term Rehabilitation and Post Acute Care program that focuses on restoring your independence so you may return to your home. Our interdisciplinary team understands your care must be as unique as you are. PCSC professionals work with you to meet your desire for independence while providing the support you need to recover and RENEW.
RENEW’s person-centered clinical, post-acute care services include:
• Therapy coverage available seven days a week
• Personal therapy evaluation in the first 24 hours of admission
• Physician-driven weekly interdisciplinary rounds
• 24/7 nursing services
• Case Management Services: prior to, during admission and post-discharge
• Wellness & Life Enrichment Programming
• Chaplaincy Services
• Scheduled Routine Follow-Up During First 90 Days Post Discharge
Therapy services include physical, occupational, speech, aquatic, and nutritional therapies.
Pathways to Success
RENEW’s Pathways to Success clinical programs are customized to your needs during Short-term Rehabilitation. Whether having experienced an accident, illness, injury, or surgery these programs help restore your ability to function with greater independence.
PCSC communities are Medicare certified and collaborate with group payment plans, ACO’s and hospital bundled payment programs. Most insurance providers require pre-certification and this can be coordinated between your health care provider and The Village at Summerville Presbyterian Community. Private pay options are also available.
